Jason Dale Fish, 32 of Jacksonville passed after a year and half battle with cancer, Wednesday, May 25 , 2011 at his residence surrounded by his loving family.

 

Survivors include his wife Tiffany Sullivan Fish, parents, Dale and Grace Fish, three sisters, April Grace Fish, Paula Mae (Robert) Lestrange, and Carrie Marie (Ken) Keene, maternal grandmother, Albertta Carswell, niece and nephews, Anna, Nicholas, Zachary, and  Nathaniel.  He was preceded in death by his paternal grandparents, Kenneth and Mildred Fish, and his maternal grandfather, Rev. Bob Carswell.

 

Memorial Services were held at 3:00 pm Sunday, May 29, 2011 in the Christian Life Church on Cahoon Road in Jacksonville, FL.

 

In lieu of flowers, the family requests that memorials be made to Haven Hospice 8301 Cypress Plaza Drive, Jacksonville, FL  32256.
