Thomas Robert Collins, 48 of Madison, died August 4, 2011 in a Tallahassee hospital.  Mr. Collins was a native of West Islip, NY, lived in Ormond Beach for 33 years and had lived in Madison since May of 2011.

 

Survivors include his mother Joan Sommers and his sisters Deidra Romono and Tammy Cruze.
